A hospital, rehabilitation team, prosthetic provider, wound-care provider, discharge planner, case manager, social worker, or another professional can introduce Lost Limb Riders as an available community resource.

With your consent, that connection can open the door to peer support, community, resource navigation, and follow-up outside clinical care. You may also contact Lost Limb Riders directly.

  • Recognize when community or lived-experience support may help.
  • Explain the option and obtain the individual’s consent before sharing information.

  • Protect privacy and follow facility requirements.
